A growing multi-location wellness practice had accumulated a substantial body of clinical knowledge around peptide therapy — protocols, dosing guidelines, contraindications, patient considerations, and category-specific research across healing, cognition, fat loss, longevity, sleep, immune support, hormone optimization, and skin health.
The problem was access. That knowledge lived in disconnected documents, PDFs, and internal references that clinicians had to manually search through during patient consultations. Finding accurate, reliable answers was slow, inconsistent across locations, and dependent on individual staff familiarity with the material.
As the practice expanded its peptide therapy offerings, the gap between what clinicians needed to know and what they could quickly access became a real operational liability.
What We Built
Milewire designed and deployed a production clinical decision support system — a RAG-powered knowledge base that allows clinicians to query the full peptide library using natural language and receive grounded, source-cited answers in seconds.
The system was built to be accurate, fast, and trustworthy. Every answer is retrieved directly from verified clinical source documents — no hallucinations, no guesswork, no generic AI responses.
Knowledge base coverage: 15 peptides across 8 clinical categories — healing and recovery, cognitive enhancement, fat loss and metabolic support, longevity, sleep optimization, immune modulation, hormone support, and skin health.
Technical architecture:
- Python and FastAPI backend with a clean REST API
- Qdrant vector database with 1536-dimensional OpenAI embeddings
- Semantic search retrieval tuned for clinical query patterns
- Static frontend deployed for direct clinician access
- GCP-based HIPAA-compliant architecture scoped for Phase 2 expansion
How We Worked
Discovery: We mapped the client's existing document library, identified the clinical categories that mattered most to their workflows, and scoped a retrieval architecture around the actual questions clinicians ask during consultations.
Build: We engineered the ingestion pipeline, chunking strategy, and embedding model selection for clinical text. Retrieval was tuned and tested against real query patterns before any user-facing deployment.
Deploy: The system went to production with a clean frontend interface accessible to clinical staff across locations. No new logins, no complex onboarding — just a query box and reliable answers.
The Outcome
Clinicians can now query the full peptide knowledge base in natural language and get accurate, source-grounded answers in seconds. Consultation support that previously required manual document searches is now instant and consistent across every location.
The system also established the foundation for a broader HIPAA-compliant GCP architecture — a three-phase roadmap that will expand the knowledge base, add patient-facing capabilities, and integrate with the practice's existing clinical workflows.
